**PLEASE DO NOT CREATE MORE THAN ONE FAMILY ACCOUNT TO REGISTER ALL OF YOUR CHILDREN FOR ATHLETIC PROGRAMS AT PRINCETON MS or PRINCETON HS!!**

Step 1: Please CLICK HERE to Print blank Physical Forms. These forms are required to be uploaded to your account so that your child can be medically cleared. (BP, PULSE, & VISION SCREENING RESULTS are required to be written on your Physical Evaluation Forms!!!)

**If you are a transfer student entering the 10th, 11th, or 12th grade you will also need to CLICK HERE to complete your child's NJSIAA Student-Athlete Transfer Verfication Form. Your child will need to be approved by the New Jersey Interscholastic Athletic Association and must be deemed academically eligible before participating in scheduled competitions.

Step 2: Please click below to print any blank forms that may apply to your child's medical health.  The forms that you print out below will be required to be uploaded as one .PDF file with your child's properly completed physical forms at the time you submit your child's online athletic registration.

PLEASE Do Not Upload Forms With Blank Sections, Missing Signatures or Missing Physical dates etc. This will prohibit your child from being medically cleared to play their sport. Once you have ALL of your forms properly completed by you and your doctor you will upload all of them as one .PDF file when you register online.

Note:  Our board approved district physician will ask you to resubmit all forms if they are incomplete or not completed correctly!!!  Emailing physical forms to our staff is NOT PERMITTED for reasons of confidentiality.   Once uploaded to your child's athletic registration account the physical will be vaild for 1 year from the date of the physical.   If your physical expires prior to the start date of your season you will need to get your child a new physical and once again upload all of the required forms.   CVS and places such as MedExpress offer same day physicals. If you are unable to afford a physical for your child please contact the school nurse at PHS or Princeton MS.

Step 3: It is highly recommended that 6th grade athletes who are 12 years old take and pass an ImPACT test for the fall season.  At PMS all 12 year old athletes trying out for a winter sport will be required to have taken and passed an IMPACT Test.  Please understand that 12 year old 7th graders will not be cleared for a winter or spring sport unless they have taken and passed an ImPact Test.  An Impact test result is valid for 2 years.   You can register for an ImPact Test by CLICKING HERE.   To Learn more about ImPACT please CLICK HERE.   Note: If you believe your child will require special accommodations an alternative ImPACT Test will be provided.  ImPact tests are now taken remotely in the comfort of your own home.

Step 4: IMPORTANT* - If you are registering your child for a sport for the first time at our high school or our middle school you will need to create a family account.  In order to do this you will need to know your child's student I.D.   You can find their student identification number by visiting PowerSchool and clicking on the child you want to register for a sport.  Your child's student identification number is located inbetween parenthesis next to their name.  If your child is coming to Princeton High School from the Cranbury School or Princeton Charter School you will receive an email from the Director of Athletics with this important information.  Please check your spam folders!!  If you need assistance with your child's student ID please contact the main office or registrar.  You can click on the school icon below to begin online athletic registration.
